Support Coordination
Support Coordinator provides assistance, guidance, and help to the participants to become more independent in achieving their goals. Support Coordinator can help to find resources and services as per the need of participant to ensure the support provided meets the requirement.
Support Coordinator’s role is to help participants understand and make proper use of the supports available in the NDIS plan.
- Connect participants with NDIS providers, community, mainstream, and other services.
- Build participants’ confidence and skills to use and coordinate your support.
Participants are able to choose both registered and unregistered support coordination providers. Support coordination can be delivered through in-person meetings, over the phone or via video based on participants’ preferences.
RayCare Australia can assist participants to build the skills they need to understand and use their NDIS plan. Our support coordinator will work with participants to ensure a mix of supports are used to increase the capacity to maintain relationships, manage service delivery tasks, live more independently, and access the community.
